Documents the office will request
- A current state-issued photo ID or driver’s license.
- A clear copy of the front of the insurance card.
- A clear copy of the back of the insurance card.
Send identity and insurance documents only through the practice-approved secure method provided by office staff. Do not attach them to ordinary email or to the routine website contact form.
Out-of-network care
Some services may be self-pay or out of network. Patients may receive an invoice or billing statement to submit to their plan for possible reimbursement. Reimbursement is determined by the patient’s plan and is not guaranteed.
